Banda de pigmentación marrón a negra de la uña, debido a la presencia de melanina en la placa ungueal. Puede aparecer como una banda única, afectando a una uña o presentarse afectando a varias. La última forma es más común en individuos de piel oscura.
Las causas potenciales son muchas, y se requiere realizar un diagnóstico diferencial para buscar una diferenciación precisa ya que existen muchas otras causas de bandas pigmentadas en la uña, incluyendo aquellas que se deben a medicamentos, fricción, trauma, cuerpo extraño y radioterapia. Otros factores endógenos incluyen déficit de vitamina B12, embarazo, enfermedad de Addison, SIDA, desnutrición, síndrome de Laugier-Hunziker y síndrome de Peutz-Jeghers.
Se podrían incluir el liquen plano, la esclerodermia, el lupus eritematoso sistémico, el carcinoma de células básales, la enfermedad de Bowen, liquen estriado e infecciones (Aspergillus, scopulariopsosis, cándida y blastomicetos).
La melanoniquia longitudinal requiere biopsia para el diagnóstico de la matriz y para descartar el melanoma.
El signo de Hutchinson es la diseminación periungueal del pigmento en los pliegues proximal o lateral.
